Per Rule 6.101 (b), Commissioners may file a formal complaint with the Executive Director when all other attempts to resolve disputes or controversies are unsuccessful. Use this form to provide all the required documentation. Upon submission, the Executive Director will acknowledge receipt and notify the alleged non-compliant state.
